New research from Salt Security's 1H 2026 State of AI and API Security report puts a specific number on something a lot of IT leaders have been quietly nervous about: 48.9% of organizations are entirely blind to machine-to-machine traffic. They know agents are running. They don't know what those agents are actually doing right now.
80% of organizations deploying autonomous AI have already caught their agents doing something they shouldn't. Unauthorized system access. Data exposure. Scope creep beyond the original permission grant. But in most cases, they only found out after it happened.
This isn't a future problem. These are AI tools running in your environment today, likely under an OAuth token someone approved months ago without much thought.
❓Why It Matters
Here's what makes this different from a regular software bug. Autonomous agents are designed to act. They don't wait for human approval. When an agent makes a mistake — or gets manipulated by a bad actor through prompt injection — it executes that mistake at machine speed.
Only 23.5% of security leaders say their existing tools are effective against agentic threats. The rest are watching agents work and hoping nothing goes wrong. That's a terrible strategy.

Assign a human owner to each agent — someone accountable for its actions, its scope, and its blast radius if something goes wrong.
If you can't name who is responsible for a given agent's behavior, that agent shouldn't be running.
