A new wave of reports this quarter says 88% of enterprises have already had an AI agent security incident in 2026. Read that again. Not "are at risk of." Have already had.
Here's what nobody's connecting. A supply chain attack on the OpenAI plugin ecosystem reportedly harvested agent credentials from 47 enterprise deployments. Six different research teams found working exploits against Codex, Claude Code, GitHub Copilot, and Vertex AI. Every single attack targeted credentials, tokens, or permissions. Not the model. Not the prompts. The keys.
And only 21.9% of organizations treat AI agents as separate identities with their own access controls. The rest are running agents under a human service account, or worse, under a developer's personal token.
❓Why It Matters
Think about what an agent does for you. It reads your email. Queries your CRM. Pushes to GitHub. Pulls from Snowflake. Creates Jira tickets. Updates Salesforce. It is, functionally, a senior IC with copy-paste access to half your business.
Now imagine that agent's auth token sitting in a config file in some forked repo. Or in an MCP server some intern stood up on a vendor's marketplace. That's where it lives today. That's the attack surface.
When the agent gets popped, you don't see a login from Belarus. You see what looks like normal automation traffic. Reading. Querying. Updating. The breach blends into the agent's own behavior, which is exactly why dwell time on these is being measured in months.

Treat every AI agent as a digital employee starting Monday. Give it a unique identity, a least-privilege role, and an off-boarding process.
If your agent is currently authenticating with a personal access token or a shared service account, rotate it this week. Then write the policy: every new agent gets its own service principal, its own scoped role, and shows up in the same access review your humans do.
